Patient Forms and What to Expect
"We put the CARE back in dental care!"
What To Expect
Comfort and convenience are the hallmarks of Dr. Calvin's office operations. Your first visit to our office establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information such as your medical history and give you time to meet your dentist. To help ensure convenience, please visit the office tab for information on appointment scheduling, in-network insurances and payment options.
Initial Oral Examination
Your initial oral examination includes a visual examination, periodontal probing and charting, diagnosis and treatment recommendations. We will need to take x-rays on all new patients, the only exception being pregnant women. Our x-rays are digital (low radiation exposure) and consist of a panoramic x-ray to enable proper diagnosis of anterior (front) and posterior (back) teeth, as well as bite-wing x-rays series for proper diagnosis of proximal decay of posterior teeth.
Oral Conscious Sedation
For patients who are overly anxious about their dental treatment, we prescribe a drug called Halcion. This is taken one hour before your appointment and puts you into a relaxed state to where you don't remember much of the appointment. The doctor is still able to communicate with you during the visit. We have had excellent results with this type of sedation. However, patients must have a driver and cannot take a cab or use a ride sharing service. As Halcion is an amnesiac, should you not remember your visit and have questions, please give us a call. We also prefer payment for services to be handled before the Halcion appointment. That enables a much smoother check out process after treatment.
Please print and complete forms to expedite your first visit. An updated medications list is very helpful, especially since some medications can affect your dental health.
